New patients accepted by referral only

Once your referral is processed we will call you to schedule the appointment, please specify the location of your preference. We appreciate a 24 hours notice for cancellations and modifications. New patients should arrive at least 15 minutes earlier than appointment to make sure paperwork is complete.

We accept all insurance plans. Payment for items not covered by insurance will be requested from you after claims are submitted. Copay is expected at the time of visit.

We will make every effort to minimize the cost of your medical care. We recognize the need to have set payment plans when needed and we are here to discuss and form a plan that works best for you!

If you have questions about your care after your visit, you can either contact our office during office hours, or send us an email on Mychart.

Medications and refills, should be requesting by phone during office hours or through your Mychart account.

If you wish to request records, please contact our office directly during office hours or send an email on MyChart.

  • Valid identification

  • Valid insurance information

  • Completed new patient questionnaire

  • List of medications

  • Home BP numbers if you check them routinely

  • We will be collecting a urine sample at your first appointment, so come hydrated!
